get-maptools simply installs libgeotiff from sources, as it predates the
inclusion of libgeotiff in most package repositories.

If the user runs it after proj.4 is installed, then all is well.

The instructions on the wiki tell you to run it after doing all other
installations. Following those instructions will get you a libgeotiff
built with proj.4 support.

I pointed out several months ago that Ubuntu (and all Debian variants) have
a version of proj.4 in their repositories that was improperly built withoug
NAD27/NAD83 datum conversion grids, so I've been recommending that anyone
who uses proj.4 for anything other than Xastir should build it from source
instead. This is not necessary for Xastir, because Xastir doesn't use
proj.4 for datum conversions, but is necessary for just about everything
else (including GDAL tools like gdalwarp).
